On Wednesday 13 October 1954, an explosion at the Collinsville mine where 40 miners were working underground killed seven and severely injured two. A Warden’s Inquiry was established to investigate the incident, followed by a Royal Commission.

The 1956 Commission findings established the disaster was caused by a gas outburst, in which the carbon dioxide leaked into the main shaft. Many miners were affected by the toxic gas.
